Travel Risk Assessment

Please note: If you are travelling to more than 2 destinations we are unable to give advice here at the Surgery. You will need to contact a travel clinic to book a consultation and any non NHS vaccinations you may require

Please ask for a copy of your vaccination history to take to your appointments

If you are travelling abroad please make sure you contact us in plenty of time to arrange any vaccinations that may be necessary. You need to contact us 6 weeks before you are due to travel in order to get the advice you need and due to some vaccination timescales. To help the Travel Nurses assess your travel needs it is important that they are in receipt of the fully completed assessment form to assess your needs and make any necessary arrangements.

If you are travelling in less than 6 weeks or travelling to multiple destinations, you will be advised to contact a private travel clinic. Please contact our reception to request a copy your vaccination history to take along to your private travel appointment.
